Forms & Deadlines

Please keep the following deadlines in mind and forward the required forms to the appropriate person as listed below.

Please be advised that the December 1 deadline for Membership, Chapter Officers and Junior Standing Forms as well as membership dues payment MUST be met in order for your chapter to be considered to be in good standing. All Junior chapters also need to submit a Fund Drive donation (minimum of $75) by July 15th.  If these deadlines are NOT met then your chapter may not have full privileges at the National Events as stated In the Jr. Standing Form.
